Create a form from scratch in Acrobat

Create a form from scratch

You can start with a blank PDF, add the required text and labels in the PDF, and then add form fields to complete the form.

  1. 1. From the Home view in Acrobat, select See all tools. From the Edit section, select Prepare a form.

  2. 2. In the Prepare a form tool, select Start from blank page > Create form.

  3. 3. The form components are displayed on the left. You can add text or any other available form component.

  4. 4. Once you complete adding form fields, save the form.

Add components in a form

You can add text, text fields, image fields, etc., using the form components listed under ADD FORM COMPONENTS in the Prepare a form tools pane. 

  1. 1. From the Home view in Acrobat, select See all tools. From the Edit section, select Prepare a form.

  2. 2. In the Prepare a form page, select Start with a blank page or Select a file to open an existing form. 

  3. 3. To add text, select the Text option and enter the desired text or information wherever you want to in your form.

  4. 4. Select the desired form components under ADD FORM COMPONENTS to add other components. Select the desired location in the form to place the selected field.

While you’re adding fields, select Preview from the Prepare a form tools pane to view how they look. Select Exit preview from the upper right to come back to the prepare a form mode.

Edit components in a form

To change existing text or images in a form, select Edit from the global bar in the upper left. All the text and image fields are enabled for editing.  

Note

Set the field properties. The field properties vary based on the type of the form field.

Add a logo or image in a form

To add a logo or image to the form: 

  1. 1. Select Edit from the global bar in the upper left

  2. 2. Under ADD CONTENT, select Image.

  3. 3. From the drop-down menu, select Choose image to select an image from your device or Select Generate image to generate an image from text.

  4. 4. From the dialog box that opens, select the image you want to add, and then select Open.

  5. 5. Place the image at the desired location. Adjust the size or location of the image as appropriate.
